Tips on hospitalization and advance directives

These tips can help prepare you and your family in the event you become seriously ill or injured and are hospitalized.  Being prepared is such a gift to your loved ones in the event something happens to you.
1. Prepare Advance Medical Directives

Advance directives are legal forms that let you have a say about how you want to be cared for if you get very sick.

Share the advance directives with your family and medical providers.Go to prepareforyourcare.org: they will walk you through how to prepare these. It is an easy process and takes little time.

2. During COVID19, if you are sick, Don’t wait until you are so short of breath that they need to intubate you on arrival. When in doubt – go to the hospital.

Take your cell phone AND charger (long cord) with you.

Take your Advance Medical Directive with you

If admitted to the hospital, designate one family member to call the hospital & nurses to get information. The medical care providers need to use their time to care for patients rather than repeat information to multiple family members.
